EllisDon Forming Ltd. and two of its supervisors have been fined a total of $130,000 following two separate roof collapses at construction sites in London, according to a report by OHS Canada. The company had been contracted to install the CANAM Hambro D500 composite floor system at both projects.
The first incident occurred on July 22, 2022, when supervisor Corey Jones was overseeing work on a penthouse rooftop. A unique roof configuration required additional cross bracing that was not clearly identified on the design drawings, resulting in it not being installed. While workers were pouring concrete, the structure shifted and collapsed, causing them to fall to the level below. No injuries were reported.
The second collapse took place on Jan. 5, 2023, under the supervision of Matthew Thompson at a residential high rise project. After a pre-pour inspection, workers began placing concrete on the 26th floor. Before the final section was poured, part of the composite floor system gave way, sending workers to the level below and resulting in four minor injuries.
The court found that the company failed to ensure that all components of the projects, including temporary structures, were adequately braced to maintain stability. Both supervisors were convicted for failing to ensure workers followed the required protective measures under Ontario’s Occupational Health and Safety legislation. Following an internal investigation, design changes were made to the CANAM Hambro D500 system. In addition to the fines, a 25 per cent victim fine surcharge was applied as mandated under the Provincial Offences Act, with funds directed to a provincial program supporting victims of crime.
